In this article, we’ll explore the services provided by Tri County Health and how it is making a positive impact on the lives of those it serves.
About Tri County Mental Health

Founded in 1983, Tri-County Behavioral Healthcare is one of Texas’s 39 community centers. The center has service locations in Conroe, Cleveland, Huntsville, Liberty, and Porter.
It is currently serving individuals with mental health conditions and intellectual or developmental disabilities across the state’s 254 counties.
As a local governmental unit, Tri-County receives sponsorship from the Commissioners Courts of Liberty, Montgomery, and Walker Counties.
Tri-County’s mission focuses on enhancing the quality of life for individuals and communities they serve.
They achieve this by delivering quality services for mental illness, substance use disorders, and developmental disabilities.
Their vision is to develop a comprehensive care system with sufficient resources to provide effective services that meet community needs.
To realize this vision, Tri-County partners with local entities to expand both new and existing resources and ensure access to culturally competent, highly trained staff.
Tri-County Behavioral Healthcare is also recognized by the State of Texas as a Certified Community Behavioral Health Clinic (CCBHC).
This certification allows them to provide integrated, culturally responsive care and recovery support for adults and families facing behavioral health issues, trauma, and chronic illnesses.
Mental Health Services Provided By Tri County
Tri County Mental Health offers the following services designed to meet the mental health needs of its clients:
1. Adult Mental Health Services
Tri-County provides essential adult mental health services, resources, and referrals aimed at promoting positive changes for each of their clients.
Among the core services, Tri-County offers tailored mental health care for eligible adults, with additional specialty services available based on assessment and program eligibility.
Each individual is assigned a recovery team to collaboratively identify helpful services and supports, working toward their specific recovery goals.
For those who may benefit from psychiatric medication, Tri-County offers medication management services.
An initial psychiatric evaluation with a prescriber establishes an effective regimen, followed by routine appointments to ensure symptom alleviation.
Case management is another service they offer, providing individuals with a case manager who helps identify needs, monitor treatment responses, and establish personalized goals.
They also offer adult psychosocial rehabilitation and skills training services focused on building essential skills to improve daily functioning.
The service frequency and intensity are determined based on individual care needs.
Peer support services are also available, provided by certified peer specialists who use their lived experiences to inspire and assist others in achieving their recovery goals.
2. Child and Youth Mental Health
Tri-County Behavioral Healthcare provides child and youth mental health services for children and teens aged 3 to 17.
These services support those who have been diagnosed with a mental health disorder and experience significant functional challenges.
Admission criteria are established by licensed clinical staff to ensure appropriate care and support.
These services are child-centered and family-focused, aiming to address both individual and family needs.
One of their primary services is “crisis, access, and referral services”, which includes an assessment to determine eligibility and guide families toward appropriate resources.
In crisis situations, this process is expedited to provide timely support.
They also offer child and teen counseling services, which provides office-based support to help youth and their families address emotional concerns like depression, anxiety, and trauma.
Case management services support families by connecting them with community resources and coordinating ongoing care plans.
They also have a community-based skills training program that helps children and youth develop skills to manage mental health symptoms effectively.
It covers important areas such as anger management, social skills, and transitional skills for adulthood.
3. Crisis Services
Tri-County offers 24/7 crisis services for individuals facing mental health emergencies, including a crisis hotline, assessment, intervention, and stabilization in the least restrictive setting possible.
For those requiring a higher level of care, voluntary and involuntary inpatient psychiatric admissions are available.
Crisis services support individuals aged 4 and above in Walker, Liberty, or Montgomery County who are experiencing a mental health emergency.
These services are available even if the person does not receive other services from Tri-County.
Crisis assessments and inpatient services are provided on a sliding scale for uninsured individuals, with fees as low as $0.
However, crisis services do not address primary substance abuse treatment or major medical care.
4. Home and Community-Based Services (HCS)
Home and Community-Based Services (HCS) is a Medicaid waiver program that provides services and supports to Texans with an intellectual disability or a related condition.
The program aims to help individuals live independently in the community. HCS services are supplemental and not intended to replace other services for these individuals.
HCS providers can offer personal assistance services, habilitation, or emergency response services through the Community First Choice (CFC) program.
These services are available to individuals who live in their own home or family home.
Individuals receiving HCS services can receive unlimited prescriptions through the Medicaid Vendor Drug Program.
These services include residential care, group homes, and host home/companion care.
They also provide respite services, day habilitation, and employment support.
Additionally, nursing services, dental care, and behavioral support are available, along with supported home living that includes transportation assistance.
Other services offered are social work, occupational therapy, physical therapy, and speech therapy, all designed to support individuals in improving their quality of life.
5. Intellectual and Developmental Disabilities (IDD) program
Tri-County’s Intellectual and Developmental Disabilities (IDD) program offers several services to individuals with intellectual disorders or related conditions.
The program provides case management services, with service coordinators helping individuals access necessary support to improve their quality of life.
These coordinators assist in areas such as crisis prevention, service planning, and coordination of medical, social, and educational services.
Intake and admission services are available for those seeking placement in state supported living centers or other IDD facilities, with eligibility assessments as needed.
Tri-County also offers continuity of care services to help individuals transition smoothly between state facilities and community placements.
Day habilitation programs, offered both at Tri-County sites and in community settings, help individuals develop essential life skills such as social interaction, and job readiness.
Supported employment services are also offered, which assist individuals in finding and maintaining employment, providing support through the job application and interview process.
Additionally, their community support services offer assistance with daily living skills, such as personal hygiene, money management, and medication self-administration.
6. Criminal Justice Services
Tri-County’s Criminal Justice Services offer a range of mental health services to individuals who qualify for mental health care under Department of State Health Services standards.
These services are available to those involved in the criminal justice system.
The Adult Criminal Justice Programs include services through the Texas Correctional Office of Offenders with Mental and Medical Impairments (TCOOMMI).
These programs are specifically for adults on felony probation in Montgomery and Liberty Counties, and adults on parole in Montgomery, Walker, and Liberty Counties.
They also serve adults on conditional bonds for mental health treatment.
Participation in the Adult Criminal Justice Programs is not open to the general public.
Individuals must be referred to the program by a probation or parole officer, clinical staff conducting mental health assessments in jail, or court officials/defense counsel.
Those who qualify for these programs are admitted regardless of their ability to pay. These services help individuals live independently in their communities.
7. The Military Veteran Peer Network
The Military Veteran Peer Network (MVPN) serves as a platform connecting organizations and individuals dedicated to supporting Texas Veterans and their families.
Through this network, veterans can stay connected, share information, post events, and develop best practices to assist in their transition to civilian life after military service.
Peer-to-peer support plays a major role in helping veterans navigate these struggles, as there is no one better suited to understand their experiences than another veteran.
Through MVPN, veterans are offered training to become peer mentors. Once trained, they are encouraged to facilitate peer groups and other forms of support within their local communities.
The Texas Department of State Health Services (DSHS) provides funding to local mental health authorities (LMHAs) to enhance resources for service members, veterans, and their families.
8. Physical Health Services
Tri-County prioritizes the overall health of its clients by offering basic medical care through the Integrated Healthcare Clinic (IHC).
This clinic is available to adults actively participating in Tri-County’s adult outpatient program.
The IHC provides services to address various health concerns, including hypertension, diabetes, high cholesterol, and other medical conditions based on individual assessments.
Primary care physicians at the clinic monitor and treat these conditions.
For added convenience, clients receive care through the Mobile Integrated Healthcare Clinic, which visits each location weekly to provide essential medical services.
